Explanation and Guide to Forms

Introduction

• The Approach to this and all Articles is consistent with our Standard System and Procedure.
• We have developed a series of Forms to simplify the following:
  • Data capturing
  • Information gathering
  • Authorisation and Approval
• Once we receive the completed form, ONLY then will we be able to execute the instructions.

Help

• Each Form has a “Help” section which is specific for that form.
• It gives a guide on how to complete each field.

• Each form has a separate “Article” link which gives further advice, guidance and background to the form in question.
• It is essential reading and we encourage clients to read this before completing the form.

Form Fields

• Each form has specific fields that are required to be completed.
• The required fields are marked with a red asterisk (*).
• All Forms must have your name and a valid email address.

Security PIN

• The 6 Digit Security PIN must be used by you, on every occasion a form is being submitted to Avar.
